The conditions observed during this site <br /> evaluation indicate that the area around the test pits is capable of supporting a STANDARD system for the <br /> ALTERATION. Plans are approved as REVISED. Please refer to the signed approved site plan dated January <br /> 18, 2019. <br /> Any deviation from the approved plans must be authorized by this office prior to installation or the permit may be <br /> voided. It is the responsibility of applicant/installer to coordinate these requirements with all others in OAR <br /> 340-071-0220. <br /> This Permit is for a system designed to serve a single family dwelling with a maximum of 4 bedrooms. Peak <br /> sewage flow into the system is limited to a maximum of 450 gallons per day,with an average sewage flow of not <br /> more than 225 gallons per day. Premature failure of the treatment system may occur if either of these flow <br /> quantities is exceeded. If,for some reason, you expect your domestic household water use may exceed these <br /> flows, it is advisable to increase the size of the treatment system. <br /> Specific Requirements <br /> TANK RISER: A watertight riser must be installed to extend to the ground surface or above. The riser lid must <br /> be gasketed and securely fastened. <br /> EFFLUENT SEWER:The effluent sewer is the pipe from the septic tank to the distribution box. The material <br /> required is 3034 or Schedule 40 pipe with 18-gauge minimum green tracer wire or green metallic tape placed <br /> along the pipe. The effluent sewer shall extend at least five(5)feet from the tank. There must be a minimum <br /> difference of eight(8)inches between the invert(flow line)of the septic tank outlet and the invert of the <br /> distribution or drop box outlet. In Equal Distribution, there must be six(6) inches of fall between the septic tank <br /> outlet and the distribution box inlet. In Serial Distribution, there must be two (2) inches of fall between the septic <br /> tank outlet and the drop box inlet. <br /> HEADER PIPE between Distribution Box and drainfield trench must measure a minimum of 4 feet, must be <br /> installed level, and bedded on undisturbed earth. <br /> DO NOT INSTALL SYSTEM DURING WINTER MONTHS (Nov. 1 —March 1) <br /> The geographical area in which the property lies is considered a risk to installation during winter months. <br /> Therefore,for this installation a-Marion County Onsite Wastewater Specialist must specifically approve soil <br /> moisture conditions at this site for installation to occur during the period from November 1st to March 1st. This <br /> may include a field visit directly prior to installation. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the Department of Environmental <br /> Quality rules. A copy of the DEQ setback requirements is available in our office and online at <br /> http://www.oregon.gov/deq/Residential/Pages/Onsite.aspx under Rules: Division 71:Table 1. <br /> THE ATTACHED SEPTIC TANK ABANDONMENT FORM MUST BE COMPLETED AND RETURNED TO OUR <br /> OFFICE. The septic system installer must complete and sign the form and attach a copy of the receipt from the <br /> septic tank pumping service for pumping the tank. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and <br /> farming activities. Other activities such as excavating or filling, driveway placement, paving or other similar <br /> activities that alter the natural soil conditions will adversely affect the soil and the septic system function and lead <br /> to premature system failure. The area must not be subjected to excessive saturation due to artificial drainage of <br /> ground surfaces, driveways, building downspouts or other sources.
