<br />Marion County Class Specification BulletinPage of <br />>>> <br />Care Coordinator <br />Class Code: <br />108 <br />Bargaining Unit: 06 - Marion County Employees <br />Association <br />MARION COUNTY <br />SALARY RANGE <br />$23.33 - $31.28 Hourly <br />OVERVIEW: <br />FLSA: EEOC: Department: <br /> Non-Exempt 05 ParaprofessionalsHealth <br />GENERAL STATEMENT OF DUTIES <br />Facilitates team meetings: develops care plans including professional and natural supports to meet the <br />intensive community-based treatment and support services needs of clients and their families; provides <br />care coordination and case management; provides direct client services to clients and their families by <br />facilitating strengths/needs assessments and referrals to appropriate agencies or services; supports the <br />development of a system of care and assists in service coordination with partner agencies. <br />SUPERVISION RECEIVED <br />Works under the general supervision of a supervisor who assigns work, establishes goals and reviews <br />the results obtained for overall effectiveness. <br />SUPERVISION EXERCISED <br />Supervision of others is not a responsibility of positions in this classification. <br />EXAMPLES OF DUTIES - DUTIES INCLUDE, BUT ARE NOT LIMITED <br />TO THE FOLLOWING: <br />1.Orients clients, families and teams to program and to wraparound values and process. <br />2.Develops Strength/Needs assessment with input from client, family and involved professional and <br />natural supports. <br />3.Completes a Functional Behavioral and develops a Crisis/Safety Plan. <br />4.Facilitates team meetings and a consensus-based collaborative decision-making process to <br />develop a service coordination plan addressing the strengths and needs of clients and families <br />across multiple domains. <br />5.Facilitates the collaborative development, periodic review and revision of a service coordination <br />plan, including a proactive safety and crisis plan. <br />6.Ensures shared accountability for all team members and is responsible for the development of <br />outcome based goals and measurement strategies. <br />7.Participates in intra-agency and interagency case staffing meetings for purposes of evaluation and <br />treatment planning for clients. <br />8.Serves as an advocate/liaison in assisting clients obtain necessary services. <br />9.Ensures that the team meetings and all supporting mental health services are provided in a <br />culturally appropriate manner, emphasizing family and client voice and choice in the decision- <br />making process. <br />https://agency.governmentjobs.com/marion/default.cfm?action=specbulletin&ClassSpecID=...8/3/2020 <br /> <br />
