Marion County Class Specification BulletinPage of <br />>>> <br />judgment by seeking consultation with a mental health specialist or public health medical staff when <br />needed and keeps the supervisor well informed of progress via verbal and written reports. <br />EXAMPLES OF DUTIES - DUTIES INCLUDE, BUT ARE NOT LIMITED <br />TO THE FOLLOWING: <br />Duties may include any of those included in the PHW 1 classification as well as the following. <br />Duties listed are examples and not all-inclusive. <br />1.Refers clients to appropriate social service agencies as needed; coordinates agency referrals; <br />provides transportation for clients to appointments if needed; communicates information regarding <br />each client's care plan and progress to other agencies involved in overall care. <br />2.Makes independent visits to clients to gather information regarding client progress against, and <br />compliance with care plan. <br />3.Provides social support for families and individuals; practices active listening skills, reflecting back <br />client needs and desires. <br />4.Communicates community perspectives and needs to colleagues in the health and social service <br />systems. <br />5.Manages medical inventory including placing and tracking orders, reconciling invoices, maintaining <br />baseline supplies. <br />6.Triages client calls and refers for appropriate care with nurse or nurse practitioner. <br />7.Makes independent visits to clients to assess client progress against, and compliance with care <br />plan. <br />8.Conducts or participates in community educational sessions with clients regarding health issues, <br />available social services and means to access; disseminates educational information. <br />9.Coordinates clinic schedules and activities. <br />10.In some job assignments, may be required to deliver medications to clients in accordance with the <br />Board of Pharmacy regulations and Nurse Practice Act. <br />11.May provide training or orientation to Public Health Worker 1 if assigned. <br />12.May assist clients with the OHP application process, including tracking and follow up and providing <br />additional referrals and resources. <br />13.Performs other related duties and responsibilities as required. <br />MINIMUM REQUIREMENTS: <br />EXPERIENCE AND TRAINING <br />1.Graduation from high school or GED; AND <br />2.Two (2) years of experience working with the culture of the client population served by the position; <br />OR <br />3.Two (2) years of college training in health occupations or social sciences; OR <br />4.Any satisfactory equivalent combination of education, training and/or experience relevant to the <br />position. <br />SPECIAL REQUIREMENTS <br />Proficiency in both the English language and the preferred language of the client population served <br />> <br />by the position. <br />If the position requires driving must possess, or obtain within 30 days of hire, a valid Class C <br />> <br />Oregon driver's license. <br />If the position requires driving must possess and maintain an acceptable driving record. <br />> <br /> If personal vehicle is used, must possess and maintain proof of personal automobile liability <br />> <br />insurance, at a minimum, in the amount required by Oregon Law \[ORS Chapter 806\]. <br />https://agency.governmentjobs.com/marion/default.cfm?action=specbulletin&ClassSpecID=...8/4/2020 <br /> <br />
