not registered from 1988 to 1993, and a fee of 535.00 per year not registered since 1994, <br />must be submitted with the Notice of Tank UST Decommissioning form. Notice to the DEQ <br />of permanent decommissioning must be submitted 30 days in advance of tank <br />decommissioning. Notice to the DEQ of intent to proceed with tank decommissioning must ' <br />be made 72 hours prior to tank removal. <br />1_2 Tank Removal and Decommissionin <br />A licensed UST Decommissioning Supervisor will provide project oversight of UST removal <br />and decommissioning activities. <br />BB&A will request a utility check for identifying buried utilities within the investigation area. <br />BB&A will also consult with county personnel and the project engineer regarding on-site <br />utilities prior to subsurface exploration. <br />Surface material will be removed as necessary to expose the top surface of the UST. Excess <br />liquids will be removed from the tank and disposed of in an authorized manner. The tank <br />atmosphere will be measured to determine if the tank is safe for opening. Determination will <br />be made on the basis of field measurements of the lower explosive limit (LEL) and oxygen <br />content. As necessary, measures will be implemented to inert the tank (i.e., reduce LEL <br />and/or oxygen content of the tank atmosphere to safe levels). <br />Excavation will be completed and the inerted tank and product lines removed from the <br />subsurface. The tank will be opened, cleaned, and transported to a disposal and/or recycling <br />facility in an authorized manner. <br />2._0 SOIL EXCAVATION AND GROUNDWATER DISPOSAL <br />~ As indicated in item 3 of the bid schedule, approximately 3,450 cubic yards of soil materials <br />are to be excavated and temporarily stockpiled on-site. The temporary stockpiles are to <br />~ bermed with bails, and lined and covered with a six (6) mil plastic sheeting. <br />Prior to undertaking soit excavation activities, BB&A will request a utility check for identifying <br />~ buried utitities within the investigation area. BB&A will also consult with county personnel and <br />the project engineer regarding on-site utilities prior to subsurface exploration. <br />Soil excavation is to be accomplished using a 40,000 pound excavator (i.e.,, John Deer 690 <br />or equivalent). Additionally, a CAT 416B 4X4 extendahoe will be used to shuttle excavated <br />material and provided additional excavation capabilities as necessary.
