She explained <br />his benefits and Mr. Heinle signed the summary of benefits indicating his <br />maximum rent supplement would be $462.00 and his moving expense <br />reimbursement would be $550 for a total claim of $1,012.00 . He <br />received a notice to vacate from her. A~~lacement Housing Payment <br />Determination form signed by Becca Reel agent for Dan Beney calculated <br />a housing additive of $462.00. <br />March 4, 1997 Ms. Andrews met Mr. Heinle at Court Street. Mr. Heinle indicated he <br />was looking for a place to move. <br />April 18, 1997 Completed RPlocation Schedule Move Claim signed by Becca Reel. <br />Moving claim determined to be $550. <br />Apri122, 1997 Notice to move on or before May 3 l, 1997. <br />Relocation Residentx Occupant Lnterview (not signed) and Relocation <br />Tncome Verification (Incomplete) <br />June 24, 1997 Mr. Heinle presented rental agreement for Jody Naff and himself at Glen <br />Creek Park Apartments effective May l, 1997 with a rental amount of <br />$495.00 plus a$55 no refundable carpet cleaning deposit. <br />Mr. Heinle submitted his Or~gon Individual Income Tax Retum 40S for <br />1996 and 1996 Federal Income Federal Income Tax Retum 1040EZ <br />in 1' W2 Wage Summarv for 1996 from Shop & Go Market and <br />Cascade Sports Camp, Inc. His total income for 1996 was $3,213. (Both <br />retums a.re dated but not signed. Mr. Heinle's rent and utilities exceed his <br />income by $411.00.) The completed Relocation Income Verification was <br />signed June 24, 1997. Mr. Heinle's occupation is student. <br />July 17, 1997 Becca Reel inspected a comparable dwelling. (I believe the property was <br />located at 1488 State # 3. The inspection is signed by both Mr. Heinle <br />and Ms. Reel.) The dwelling type indicates need for 1 adult male and 1 <br />adult female. The Relocation Rgplacement Housing C_1~ determined a <br />rent supplement of $462.00 plus a$55.00 non refundable fee. <br />August 8, 1997 Letter from NIr. Heinle indicating his student loans were paid direct to <br />Willamette University and a11 his living expenses were paid from <br />previously submitted W-2 forms. <br />September 15, 1997 Elyn Lyon interviewed Mr. Heinle on the telephone.
